Lone Star House Call Physicians provides in-home primary care physician services to an area of Texas that extends from Huntsville in the north to Santa Fe in the south, from Liberty / Crosby in the east to Katy in the west, and everything in between including Houston and surroundings.

 

Primarily we serve elderly and/or disabled, homebound patients with multiple chronic diagnoses. The primary care physician will provide monthly in-home or tele-visits for continuity of care, medication management and healthcare coordination. In addition, patients have access to telehealth services to the physician or nurse for a range of illnesses that may require treatment between regularly scheduled physician visits. Our goal is to reduce taxing transfers to physician offices, as well as decrease hospital admissions. Your physician may order home health services, durable medical equipment, labs/imaging, and make referrals to specialists when needed.
